Note: Injuries occuring on game day are to be documented by your coach/manager through the online match report system. If St. John's Ambulance are in attendance they will also complete their own injury report and give you a copy. 

 

Injuries that occur at training must be reported to the Club Secretary via email by the coach/manager.

 

The claim form is available from SLE Australia's website, or contact the Club Secretary for a form. On completion of the form, the Club Secretary will sign and forward onto SLE.
